Tennessee Governor Bill Lee has ordered a third-party review of Christa Pike’s failed execution and paused executions across the state for the rest of 2026. [1] [2]
The pause affects Gary Wayne Sutton, whose execution was scheduled for 3 December. His was the only other Tennessee execution scheduled this year. [1] [2]
The Associated Press reported that Lee had indefinitely postponed both Pike’s execution and the state’s other scheduled execution. [2]
Tennessee Governor Bill Lee has ordered a comprehensive third-party review of the failed execution of Christa Pike. He said it should “determine exactly what occurred” after Pike survived a lethal-injection procedure on Wednesday, 30 September. [3] [7]
The review follows differing accounts of the procedure. Tennessee’s Department of Correction said it followed every step of its execution protocol and that the protocol allowed no further procedures. Pike’s lawyers alleged problems including difficult vein access, blown veins and degraded pentobarbital. [5] [6]
Executions in Tennessee have been halted for the rest of 2026. The Associated Press reported that Pike’s execution and the state’s only other execution scheduled this year had been postponed indefinitely. [3] [2]

Governor Bill Lee announced the change on Saturday, the BBC reported, citing CBS News. Strada, Tennessee’s corrections commissioner, is set to leave following Wednesday’s unsuccessful attempt to execute death-row prisoner Christa Pike. [8]
In a statement to CBS quoted by the BBC, Strada said stepping down was “in the best interests” of Tennessee. He called the continuing investigation into the incident “entirely appropriate and necessary”. [8]
The investigation into Pike’s attempted execution is continuing. A date for Strada’s departure has not been specified. [8]
Tennessee corrections commissioner Frank Strada will leave his post this month after Governor Bill Lee accepted his resignation following the failed execution of Christa Pike. Lee has ordered an independent review and halted further executions; an interim commissioner is to be named later. [10] [11]
Pike survived the 30 September attempt after receiving two doses of pentobarbital. Her lawyers said on 2 October that she was critically ill, unconscious and on a ventilator, with significant injuries to both arms. They believe problems with intravenous lines affected how the drug entered her body. [11] [12]
Strada said he believed the department had followed its established protocol and that stepping aside was in Tennesseans’ best interests during the review. Pike’s lawyers described what happened as a systemic failure extending beyond one person. [10] [11]
Her lawyers have also pointed to five problematic execution procedures under Strada’s oversight. Three Arizona executions in 2022 were completed; two Tennessee attempts in 2026, involving Tony Von Carruthers and Pike, did not result in executions. The Death Penalty Information Center classifies the Arizona cases as “botched”, a term it also uses for completed executions involving serious problems or delays. [9] [13] [15] [16] [17]
The five cases raise questions about execution procedures, but they are not five attempts in which the prisoner survived. AP reports, citing the corrections department, that four people have been executed in Tennessee since 2025. The cause of Pike’s failed execution and any individual responsibility remain to be determined. [11]
