The Dutch Safety Board (OVV) is investigating an explosion at Gunvor Energy’s oil-products refinery and storage site in Rotterdam, where one person died and five were injured. The blast happened on 13 August during work to disconnect pipes as part of decommissioning an installation. [1]
Six people were working at the tank park at the time. The OVV says it will examine how the parties involved managed safety when opening pipelines. It says the investigation is not intended to determine guilt or liability, but to draw lessons for other companies carrying out similar work. [1]
The Netherlands Labour Authority is also investigating the incident. It is not yet clear when that investigation’s findings will be ready. [1]
