Yorkshire Ambulance Service is investigating two possible instances in which staff accessed or tried to access medical information about William Chapman, who was killed by a tiger at Yorkshire Wildlife Park, the trust says.
Chapman, 24, died after entering the tiger’s enclosure at the park near Doncaster on Saturday. An inquest heard on Thursday that he died from “multiple traumatic injuries and animal interaction”.
The trust said its audits identified the two potential incidents after Chapman’s death. It said it was investigating the circumstances and the actions of the staff involved, and had reported the matter to the Information Commissioner’s Office.
A trust spokesperson said the access may have been “without a legitimate reason”. The spokesperson said Chapman’s family had been informed and would be kept updated. The trust also offered condolences to his relatives.
